Tenet Healthcare's Nacogdoches Medical Center is seeking a Registered Nurse Case Manager to coordinate care across the patient journey, promote timely discharge, and reduce readmissions. You will collaborate with physicians, families, and care teams to ensure medical necessity and appropriate resource use.

Qualifications

• RN license in good standing and current state-specific registration.
• 2 years of acute hospital patient care experience required.
• ACM certification preferred.
• Strong organizational, communication and leadership abilities.

Responsabilités

• Accurate medical necessity screening and submission for review.
• Care coordination across transitions and resources.
• Transition planning assessment and reassessment.
• Lead and facilitate multi-disciplinary care conferences.
• Documentation in the Tenet Case Management system.
• Identify and refer complex patients to Social Work Services.
• Communicate plan of care with patients and families.
