Job summary

SolutionHealth's Elliot Hospital is seeking an RN Case Manager to provide clinically-based case management, coordinate care, and ensure safe transitions for patients across settings. You will collaborate with the health care team to identify resource needs and create effective discharge plans.

Qualifications include a BSN (or pursuing one), 3–5 years of case management experience, CCM/ACM preferred, and proficiency with EMR; on-the-job training is available for experienced nurses.

Qualifications

• Bachelor’s degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• 3–5 years of case management-related health care experience.
• CCM or ACM preferred.
• Excellent clinical documentation and EMR usage preferred.

Responsibilities

• Assesses discharge planning needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization Reviews.
