• Care coordination and transition planning assessment and reassessment.
• Implementation or oversight of the transition plan.
• Leading and facilitating multi-disciplinary patient care conferences and managing disputes.
• Identifying and referring complex patients to Social Work Services.
• Other duties as assigned.
