Ellis Medicine is seeking an experienced RN Case Manager to oversee case management for designated units, ensuring medical necessity and coordinating discharge planning with the interdisciplinary team. You will work closely with physicians, social workers, and care coordinators to optimize patient outcomes and streamline transitions across care settings.

Qualifications

• Current license or valid permit to practice professional nursing in New York State.
• Associate's degree in nursing required; Bachelor's preferred.
• Two years inpatient hospital experience preferred.
• Previous case management, utilization review, and discharge planning experience preferred.

Responsibilities

• Discharge planning: assess needs, coordinate referrals, establish discharge plan and timelines.
• Utilization management: ensure charted care aligns with InterQual/CMS guidelines; address barriers to continued stay.
• Collaborate with physicians, social workers and interdisciplinary team to ensure safe care transitions.
• Document thoroughly in medical records; participate in interdisciplinary rounds and care coordination meetings.
• Coordinate transfers between hospitals and engage Social Work for complex discharge planning needs.
