Job summary

CHS Career Site is seeking a Care Manager - RN responsible for coordinating discharge planning, transitions of care, and case management to achieve optimal patient outcomes. You will review records, collaborate with IDT, and ensure compliance with standards.

Ideal candidates hold an RN license with 2–4 years of clinical nursing experience in hospital/home health/nursing facility settings and may have ACM/ CCM or BLS credentials. Strong communication and documentation skills are essential.

Qualifications

• Nursing degree required (state licensure).
• 2-4 years of clinical nursing experience in hospital/home care/nursing facility.
• ACM/ CCM preferred.
• BLS preferred.

Responsibilities

• Coordinate discharge plans and transitions of care for patients.
• Review medical records for appropriateness and medical necessity.
• Collaborate with interdisciplinary teams to plan and implement care.
• Communicate with patients, families, and providers about discharge options.
• Maintain accurate documentation of case management activities and referrals.
• Refer cases to physicians or managers when criteria are not met.
• Facilitate transitions with community agencies for post-hospital needs.
