Job summary

Ascension is seeking a Nurse Case Manager to coordinate the interdisciplinary plan of care for patients from admission through discharge within the Case Management department. You’ll monitor medical necessity, engage with the care transition team, and ensure smooth post-acute arrangements.

Qualified candidates hold a Wisconsin RN license (or multi-state Compact), with BLS preferred. The role emphasizes advocacy, resource optimization, and collaboration with payers to support patient outcomes.

Qualifications

• Registered Nurse license in Wisconsin or multi-state Compact.
• Case Manager credential (CCMC) preferred/required.
• BLS Provider credential preferred (AHA/ARC accepted).

Responsibilities

• Coordinate care plan from admission to discharge.
• Monitor medical necessity and level of care via assessments and records.
• Collaborate with care transition team for plan implementation.
• Serve as liaison for post-acute services and documentation.
• Collaborate with payers for benefit optimization.
• Advocate for efficient use of clinical resources.
