Job summary

AnMed in Anderson, South Carolina, is looking for a Case Manager to join the Care Coordination team. The role focuses on assessing care needs, planning, coordinating, and evaluating services to support patients and families in accessing resources across care settings.

You will collaborate with physicians, nurses, social workers, and other professionals to optimize transitions, resource use, and patient outcomes through detailed care planning and thorough documentation.

Qualifications

• BSN and SC Registered Nurse license required; BSN must be completed within 48 months if not yet.
• Excellent interpersonal, negotiation, problem-solving, organizational and time-management skills; ability to work independently and in a team.

Responsibilities

• Collaborate with multidisciplinary team to achieve timely, appropriate patient management.
• Use RN process to focus clinical activities and outcomes.
• Support resource utilization and length of stay optimization.
• Identify high-risk patients and design acute and post-acute plans with the team.
• Interact with patients and community to ensure continuity of care and document plans across care settings.
• Maintain and document accurate case records and manage documentation systems.
