Job summary

Allegheny Health Network is seeking an experienced Registered Nurse to coordinate care across transitions and levels. You will apply established care management practices to support efficient resource use and optimal patient outcomes in a hospital setting.

The role requires a nursing degree, PA RN licensure (or multi-state eNLC), and strong communication and collaboration skills to work with physicians, providers, and families. This is a full-time, on-site position in the Pittsburgh region.

Qualifications

• Minimum Bachelor’s Degree in Nursing or Nursing Diploma with 6 years of relevant experience.
• 3 years in a clinical nursing role.
• Current PA RN licensure or multi-state licensure via eNLC.
• Knowledge of care management and utilization review concepts.
• Transition planning and community/facility resource awareness.

Responsibilities

• Assess patient needs for effective transition of care planning.
• Collaborate with care team to communicate continuum of care plans.
• Document data to monitor interventions and outcomes.
• Obtain pre-certifications and post-acute authorizations as required.
• Identify denial trends and support action plans with teams.
