Qualifications

• Bachelor’s degree in Nursing or equivalent with nursing diploma or 6 years of relevant experience.
• Current PA RN license or multi-state license via eNLC.
• 3 years in a clinical nursing role.
• Nationally recognized case management certification preferred.

Responsibilities

• Completes comprehensive assessment of patients and families, including social determinants.
• Identifies barriers and analyzes risk to plan safe transitions and care coordination.
• Coordinates clinical care with patient, family, providers, and team.
• Advocates for patients and families with providers, payers, and post-acute partners.
• Educates patients and families on care, documentation, and utilization principles.
• Pursues professional development through certification, mentoring, or committees.
• Adheres to hospital and clinical policies and procedures.
