Job summary

Tenet Healthcare is seeking an RN Case Manager at DMC Huron Valley-Sinai Hospital to coordinate patient care across the continuum. You will support timely throughput, appropriate levels of care, and discharge planning, while leading multi-disciplinary conferences and ensuring compliant documentation.

The role requires a nursing graduate with 2 years of acute hospital experience and 1 year in case management. RN licensure in the practice state is required; ACM preferred.

Qualifications

• Required: Graduate of an accredited school of nursing.
• Preferred: Academic degree in nursing (bachelor's or master's).
• Required: 2 years of recent adult acute hospital patient care experience; 1 year case management and discharge planning experience.
• Preferred: Acute hospital case management experience.
• Required: RN; active license to practice in state(s) covered.
• Preferred: Accredited Case Manager (ACM).
• Required skills include organizational, verbal and written communication, leadership/coordination, critical thinking and computer literacy.

Responsibilities

• Facilitate care along the continuum through effective resource coordination.
• Assess patient for transition needs to promote timely throughput and safe discharge.
• Lead and facilitate multi-disciplinary patient care conferences.
• Manage and document transition plans and referrals to other departments.
• Maintain accurate patient demographics and insurance information and ensure timely documentation.
