Qualifications

• Graduate of an accredited school of nursing.
• 2 years of recent adult acute hospital patient care experience.
• 1 year case management or discharge planning experience.
• Currently licensed RN for the state(s) covered.

Responsibilities

• Accurate LOS screening and submission for Physician Advisor review.
• Leading and facilitating multi-disciplinary patient care conferences.
• Care coordination and transition planning assessment.
• Timely, complete and concise documentation.
