Tenet Healthcare is seeking an RN Case Manager to coordinate care across the continuum, ensuring medical necessity, appropriate utilization of resources, and timely transitions for discharge. The RN will lead and participate in multidisciplinary conferences and ensure accurate documentation and communication with patients, families, and clinicians.

Requirements include graduated nursing education and an active RN license; bachelor’s/master’s in nursing are preferred.

Qualifications

• Requires graduate of an accredited school of nursing.
• Active RN license in the state of practice or policy.
• Preferred ACM certification.
• Experience in acute hospital patient care and case management is valued.

Responsabilités

• Screen medical necessity and coordinate care across transitions.
• Lead and participate in multi-disciplinary care conferences.
• Coordinate transitions to appropriate levels of care and discharge planning.
• Document thoroughly in Case Management systems and maintain accurate demographic and insurance data.
• Identify and refer complex patients to Social Work and other services.
• Communicate care plans effectively with patients, families and providers.
