Qualifications

• Graduate from an accredited school of nursing.
• Two years of acute hospital patient care experience.
• License to practice as a Registered Nurse in the State of Michigan.
• Accredited Case Manager (ACM) preferred.
• Must complete Tenet InterQual education course within 30 days of hire and demonstrate competency.

Responsibilities

• Accurately screen for medical necessity and submit for physician advisor review.
• Coordinate care and plan transitions to promote timely discharge and prevent readmissions.
• Lead and participate in multi-disciplinary patient care conferences.
• Ensure timely documentation in the Case Management system and maintain accurate demographics.
• Collaborate with physicians, staff, payors and families to support the patient plan.
