Job summary

Advanced Health Care seeks an RN Case Manager to support the Director of Nursing and facilitate smooth, safe discharges for residents. You will coordinate with the IDT to set discharge dates, complete paperwork, arrange DME and transportation, and educate families.

This full-time, on-site role requires strong communication, confidentiality, and knowledge of resources for older adults. Weekend/holiday coverage is occasional.

Qualifications

• Valid RN license in the state where work is performed.
• Minimum 2+ years in discharge planning or care coordination.
• Knowledge of community resources for older adults.
• Familiarity with skilled nursing facilities.

Responsibilities

• Communicate discharge planning effectively.
• In coordination with IDT set discharge date.
• Complete all required discharge paperwork.
• Coordinate DME needs.
• Send and communicate referral information.
• Coordinate transportation on day of discharge.
• Provide education.
• Complete required discharge documentation.
• Review Quality Indicator reports and implement corrective actions.
• Analyze quality assurance reports and present to QA Committee.
• Participate in interdisciplinary team meetings.
• Assist on the floor as needed.
• Maintain confidentiality of resident and facility information.
• Identify safety hazards and educate staff on preventive measures.
