Job summary

UCI Health is seeking an experienced care coordination RN to manage case management, utilization review, and discharge planning in line with evidence-based practice.

The role requires a CA RN license, 5+ years clinical experience including case management, and familiarity with Joint Commission and CMS requirements. Strong communication and interdisciplinary collaboration are essential.

Qualifications

• Current CA RN license and maintain current.
• Minimum five (5) years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a Bachelor's degree in Nursing.
• External candidates must have a Bachelor's degree in Nursing or above
• Must possess the skills, knowledge and ability essential to the successful performance of assigned duties
• Must possess complete understanding of acuity levels for specific patient population
• Must demonstrate customer service skills appropriate to the job
• Familiarity of Joint Commission, CMS, CDPH and CCS requirements
• Excellent written and verbal communication skills in English
• Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
• Ability to facilitate and lead interdisciplinary rounds
• Ability to establish and maintain effective working relationships across the Health System

Responsibilities

• Coordinate patient care activities to conform to evidence-based practice and regulatory/payor requirements.
• Integrate functions typically considered case management, care coordination and utilization management.
• Conduct discharge planning assessments.
• Lead interdisciplinary rounds and collaborative care planning.
• Maintain relationships across the Health System to optimize patient outcomes.
