Qualifications

• Graduate of an accredited School of Registered Nurses; BSN preferred, MSN encouraged.
• Minimum of five years of experience in the care of an assigned patient population.
• Current registration to practice as a Registered Nurse in the applicable state; Case Management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
