Job summary

Advocate Health’s Case Manager (Part-Time) coordinates care for defined patient populations at Lexington Medical Center. This RN position emphasizes advocacy, communication, and collaboration to ensure continuity of care and cost-effective, high-quality outcomes.

First shift, 20 hours per week, with benefits eligible. Under Director supervision, you will work with physicians, nurses, and allied staff to educate and support care coordination, leveraging nursing processes to optimize patient

Qualifications

• Graduate of an accredited School of Registered Nurses.
• Current RN license in applicable state.
• Minimum of five years in care of defined patient populations.
• Participation in case management and utilization review encouraged.
• Case Management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborate with physicians, nurses, rehab, pharmacy, and other disciplines to coordinate care.
• Educate, consult, and assist in managing patient populations to achieve high quality outcomes.
• Engage daily with the case management team to ensure timely achievement of expected outcomes.
• Be available by pager, digital phone, and e-mail when not on campus.
