Job summary

Atrium Health Wake Forest Baptist is hiring a Case Manager in Winston-Salem. You will use the nursing process to develop, implement, and evaluate case management outcomes for defined patient populations, coordinating with a multi-disciplinary team to ensure quality and cost-effective care.

The role requires RN licensure in the state, with BSN preferred and MSN encouraged. Shift is Monday–Friday, 8 hours per day, 40 hours per week, with a pay range of $38.20 to $57.30 per hour.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses; BSN preferred, MSN encouraged.
• Minimum of five years of experience in the care of the defined patient population.
• Current RN license in the applicable state; Case Management certification encouraged.

Responsibilities

• Utilizes the nursing process to develop, implement, and evaluate case management outcomes.
• Collaborates with physicians, nurses and support staff to coordinate patient care.
• Educates, consults, and assists in managing identified patient populations for high-quality, cost-efficient outcomes.
