Job summary

Henry Ford Hospital - Detroit Main Campus is seeking a Registered Nurse Case Manager for a contingent, day‑shift role. The Case Manager‑RN coordinates comprehensive patient care, supports transitions between care levels, and works closely with patients, families, and the care team to optimize outcomes.

The position requires strong clinical judgment, active Michigan RN license, and a minimum of three years in acute care; CCM certification within one year is preferred.

Qualifications

• Active RN license in Michigan.
• Minimum of three years of recent acute care nursing experience.
• Experience in case management preferred.
• Strong organizational and time‑management skills.
• Proficiency in electronic health records (EHR).

Responsibilities
