Responsibilities

• Assumes responsibility for caseloads consisting of patients in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care in accordance with the physician orders and patient needs.
• Provides direct patient care and adjusts nursing care processes as needed; collaborates with family and staff.
• Performs ongoing assessment of patient response to treatments and updates the plan of care as indicated.
• Completes documentation including visit assessment and orders promptly and accurately.
• Communicates with physicians to confirm and update patients' plan of care and provides status updates.
• Facilitates discharge planning and coordinates with patient/family and care team for continuity of care.
• Participates in chart audits and quality improvement activities to enhance service quality.
• Maintains accurate clinical and patient records per agency, state, and federal guidelines.
• Demonstrates age-appropriate care and knowledge of growth and development across the lifespan.
