Responsibilities

• Assumes caseloads in specialty areas and conducts initial assessments.
• Establishes the plan of care aligned with physician orders and patient needs.
• Provides direct patient care and adjusts nursing care plans as needed; advises family and staff.
• Monitors patient responses to treatments, updates care plans, and documents in records.
• Communicates with physicians to confirm and update the plan of care; ensures ongoing team communication.
• Facilitates discharge planning, coordinating with patient/family, physician, social worker and staff.
• Participates in chart audits and quality improvement; supports process improvements to raise care quality.
• Maintains accurate clinical and patient records per agency, state, and federal guidelines.
