Job summary

Aurora Health Care is seeking an experienced Registered Nurse to provide comprehensive patient care on an on-going basis. You will perform visits, assess patient needs, and update plans of care in collaboration with the interdisciplinary team.

Requires an active RN license in Wisconsin and BLS certification upon hire, with the ability to adapt care for patients across the lifespan. This is an on-site position with the hospital/clinic setting.

Qualifications

• RN licensure required; BLS (AHA) upon hire.
• Completion of an accredited nursing program.
• Ability to assess patient data and make sound clinical judgments.

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts physical, psychosocial and environmental assessments and updates the care plan.
• Communicates with physicians to confirm and update treatments and relays changes to the RN Case Manager.
• Provides ongoing assessment of patient response to treatments and updates the care plan.
• Partners with the RN Case Manager for discharge planning and uses community resources for continuity of care.
• Accurately documents and updates patient records per guidelines.
• Communicates problems clearly to physicians and RN Case Manager with clear plans and goals.
• Follows policies and regularly monitors changes to them.
• Performs patient positioning and lifting using proper technique.
• Demonstrates age-specific knowledge and tailors care to patient development and needs.
