Job summary

Ascension Health in Wisconsin is seeking a dedicated Case Manager to coordinate the interdisciplinary plan of care from admission through discharge within the Case Management department. You will monitor medical necessity and level of care, advocate for patient resources, and liaise with post-acute services and payers to optimize outcomes and ensure seamless transition across care settings.

This role requires RN licensure and CCMC certification preferred; a Diploma in Nursing is required and BLS

Qualifications

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• BLS Provider. American Heart Association or American Red Cross accepted.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Responsibilities

• Coordinates the overall interdisciplinary plan of care for patients from admission to discharge.
• Creates plan for care across the continuum and advocates for resources and removal of barriers for the patient.
• Monitors patients' medical necessity and level of care through assessments and ongoing evaluations.
• Maintains dialogue with supervisor and care transition team members to ensure implementation and reevaluation of care plan.
• Acts as liaison between patient and post-acute services for medical record documentation and services related to diagnoses and treatment options.
• Collaborates with payers for benefit optimization and advocates for efficient utilization of clinical resources.
