Job summary

Desert Willow Post Acute is seeking a qualified case manager to join our team. The role focuses on coordinating patient care across residents, families, and healthcare providers, ensuring cost-effective, high-quality plans of care.

Qualifications include a Bachelor’s Degree in Nursing or Social Work, RN license preferred, two years of clinical nursing experience, and knowledge of Medicare/Medicaid. CCM certification a plus; PCC knowledge preferred.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required or equivalent.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• PCC knowledge preferred; CCM certification a plus.

Responsibilities

• Communicate regularly with residents and families about plans of care and treatment protocols.
• Attend and participate in morning meetings to coordinate care.
• Answer residents’ questions regarding care, treatment plans, and illness progression.
• Monitor and adjust residents’ statuses based on changing needs.
• Organize and prioritize daily work and complete required documentation.
• Perform utilization review to provide cost-effective care; coordinate referrals and admissions/discharges.
• Maintain professional, respectful interactions with residents, families, staff and partners.
