Job summary

Tenet Desert is seeking a Clinical Support Specialist (RN/Case Management) to facilitate ongoing care coordination and transitions for patients. The role emphasizes psycho-social assessment, discharge planning, and collaboration with physicians, families, and other departments to ensure timely throughput and safe readmissions prevention.

The position requires a Master of Social Work (MSW) with ACM preferred and typically 2+ years of acute hospital experience.

Qualifications

• Master of Social Work (MSW) is required.
• Accredited Case Manager (ACM) preferred.
• Minimum of two years acute hospital experience preferred.

Responsibilities

• Facilitate care along a continuum through resource coordination to promote optimal health and appropriate utilization of resources.
• Assess patients for transition needs and identify those at risk for readmission.
• Conduct complex psycho-social assessments and interventions for timely throughput and safe discharge.
• Support transition management and patient satisfaction.
• Coordinate care and communicate with physicians, families, and other departments.
• Provide departmental reports and participate in staff hiring/training as needed.
• Ensure documentation accuracy in Tenet Case Management systems.
