Job summary

Desert Willow Post Acute is seeking a Case Manager with a strong clinical nursing background to lead care coordination for residents. You will develop and implement care plans in collaboration with families, physicians and payers, overseeing admissions, discharges, and referrals with a focus on cost-effective, high-quality care.

The role requires a Bachelor’s in Nursing or Social Work, and an RN license is preferred (LVN/LPN acceptable).

Qualifications

• Bachelor’s degree in Nursing or Social Work is required/preferred.
• RN license preferred; LVN/LPN acceptable.
• At least 2 years of clinical nursing experience preferred.
• Knowledge of Medicare/Medi-Cal/Medicaid benefits.
• PCC knowledge preferred; CCM certification a plus.
• Ability to maintain documentation and charting accurately.

Responsibilities

• Coordinate care plans with residents, families, physicians, and payers.
• Oversee admissions and discharges and referrals to/from the facility.
• Perform clinical assessments and care planning.
• Ensure timely documentation and utilization review activities.
• Collaborate with disciplines for cost-effective, quality care.
