Job summary

DESERTWILLOW is seeking a Nurse Case Manager to collaborate with residents, families and the care team in Las Vegas. You will develop and implement care plans, coordinate services, and ensure cost-effective, high-quality patient care.

The role requires a nursing degree with RN license preference; LVN/LPN OK. Minimum two years of clinical nursing experience and knowledge of Medicare/Medicaid programs are valued. This is an on-site position in a healthcare facility.

Qualifications

• Bachelor's Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN license acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.

Responsibilities

• Communicate plans of care with residents and families, including PT/OT and other treatments.
• Attend morning meetings to coordinate care with the team.
• Answer residents’ questions about care, plans, and illness progression.
• Monitor and adjust resident statuses based on changing needs.
• Organize daily work by assessing new, current, and discharge residents’ needs.
• Complete documentation and required paperwork in a timely manner.
• Coordinate care with residents, providers, facilities, and third‑party payers.
• Oversee admissions and discharge activities; coordinate referrals as needed.
• Relate positively with residents, families, staff, and professionals to promote cooperation.
• Maintain professional, honest interactions with residents and staff.
