Job summary

DESERTWILLOW is seeking a registered nurse/care manager to collaborate with residents, families, and healthcare providers to develop and implement comprehensive care plans. You will coordinate admissions, discharges, and referrals while maintaining accurate documentation and ensuring cost-effective, high-quality care.

The role requires strong communication, critical thinking, and the ability to work with diverse teams. RN/licensure and experience with Medicare programs are preferred.

Qualifications

• Bachelor's Degree in Nursing or Social Work required; RN license preferred.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.
• PCC knowledge preferred.
• CCM certification a plus.

Responsibilities

• Communicate Plans of Care and treatment protocols with residents and families.
• Attend morning stand-ups and facilitate team communication.
• Monitor resident status and adjust plans based on needs.
• Coordinate admissions, discharges, and referrals.
• Accurate charting and timely documentation.
• Perform utilization review to ensure cost-effective care.
