Qualifications

• Bachelor's Degree in Nursing or Social Work is required or preferred.
• RN license preferred; LVN/LPN acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid benefits is desirable.
• CCM certification is a plus; ongoing licensure is necessary.
• Continuing education/licensing must be maintained.
• PCC knowledge is a plus.

Responsibilities

• Utilize clinical expertise to plan and coordinate patient care with residents, families, and providers.
• Communicate plans of care and treatment updates to residents and families.
• Attend morning stand-ups to facilitate team communications.
• Complete accurate documentation and charting in a timely manner.
• Oversee admissions, discharge activities, and referrals to the facility.
• Coordinate care with payers, facilities, and third-party providers.
