Job summary

Canyon Vista Post Acute is seeking a qualified nurse/case manager to develop, coordinate and monitor plans of care for residents. Collaboration with families, care teams, and payers is essential to deliver high-quality, cost-conscious care.

Required are a nursing degree with an RN license preferred, 2+ years clinical nursing experience, and knowledge of Medicare/Medicaid programs. CCM certification is a plus; strong documentation and communication skills are expected.

Qualifications

• Bachelor’s Degree in Nursing or Social Work; RN license preferred.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• CCM certification a plus.
• Must maintain continuing education/licensing and stay in good standing.

Responsibilities

• Communicate Plans of Care with residents and families.
• Attend morning meetings to coordinate care with the team.
• Explain care plans and progress to residents.
• Monitor residents’ status and adjust care as needed.
• Complete documentation on time and accurately.
• Coordinate admissions and discharges.
• Coordinate referrals with providers and payers.
• Perform clinical assessments and care planning.
