Job summary

Desert Willow Post Acute seeks a dedicated professional to utilize clinical expertise in coordinating care for residents. You will collaborate with families, physicians, and payers to implement comprehensive plans of care and ensure cost-effective services.

You will conduct assessments, develop care plans, and manage referrals, with emphasis on timely charting and interdisciplinary communication to support Residents and their families.

Qualifications

• Bachelor’s degree in Nursing or Social Work and active RN license preferred.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.
• CCM certification is a plus; maintain licensure and education.
• Maintain good standing with the Department of Public Health at all times.

Responsibilities

• Perform clinical assessments, care planning, and direct coordination of medical services.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Communicate with residents, families, and care team about plans of care.
• Conduct utilization review for cost-effective, appropriate care.
• Document accurately and complete paperwork in a timely manner.
