ScionHealth's Seattle First Hill hospital seeks a Registered Nurse for care coordination across the ICT team, discharge planning, and utilization management. You will advocate for patients, collaborate with physicians, payors, and families, and ensure efficient care transitions from admission to discharge.

Requirements include an accredited RN program, preferably BSN, with 2 years of healthcare experience. Certification in Case Management is a plus; English fluency and strong communication are

Competenze

• Graduate of an accredited RN program.
• BSN preferred; MSW/BSW with state licensure acceptable.
• Two years healthcare experience preferred.

Mansioni

• Coordinate care with the Interdisciplinary Team and physicians.
• Monitor patient stay to optimize care and resource use.
• Perform comprehensive discharge planning and patient education.
• Conduct medical necessity reviews and payer communications.
