Job summary

ScionHealth is seeking a Case Manager to coordinate patient care across the continuum, ensuring efficient resource use and high-quality outcomes. You will work with the ICT team to assess needs, plan care, and drive successful transitions from hospital to post-acute settings.

Candidates should have a Nursing degree with case management experience, strong communication, and knowledge of reimbursement and external review requirements.

Qualifications

• Knowledge of government and non-government payor practices, regulations and reimbursement.
• Experience with discharge planning and care coordination is preferred.
• Fluent English communication and strong documentation skills.
• Ability to advocate for patients and coordinate across disciplines.

Responsibilities

• Coordinate and facilitate patient care through the Interdisciplinary Care Transitions team.
• Monitor patient stays for effective care coordination and resource utilization.
• Participate in care rounds and conferences to review goals and post-hospital needs.
• Provide comprehensive discharge planning and ensure timely reviews with payors.
