Job summary

ScionHealth seeks a care coordinator to optimize patient transitions across the continuum of care. You will collaborate with the ICT team to plan and monitor care, ensuring efficient use of resources and timely discharge planning.

Requirements include nursing education and experience in case management, utilization review, or discharge planning. Strong communication and English proficiency are essential for coordinating with patients, families, and providers.

Qualifications

• Knowledge of government and non-government payor practices and regulations.
• Understanding of Medicare benefits and contracts.
• Familiarity with accreditation standards and compliance.
• Demonstrated critical thinking, prioritization and time management.
• Basic computer skills including Word and Excel.
• Excellent verbal and written communication with stakeholders.

Responsibilities

• Coordinate clinical and psychosocial activities with interdisciplinary team.
• Monitor patient stays for effective care coordination.
• Refer high risk patients for additional support.
• Serve as patient advocate and educate families.
• Collaborate on care plans and discharge planning.
• Conduct comprehensive discharge planning and follow-up.
• Perform utilization reviews and ensure appropriate resource use.
• Participate in hospital committees as needed.
