Job summary

ScionHealth is seeking a skilled Case Manager to coordinate patient care across the continuum with the ICT team. The role emphasizes collaboration, assessment, and plan implementation to ensure cost-effective, high-quality outcomes.

You will work with clinicians, payors, and families to optimize resource use, facilitate safe discharges, and support patients through transitions while upholding regulatory standards.

Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance processes and contracts.
• Knowledge of accreditation standards and compliance requirements.
• Ability to demonstrate critical thinking, prioritization and time management.

Responsibilities

• Care Coordination with Interdisciplinary Team and Physicians.
• Monitor all areas of patients' stay for effective care coordination.
• Refer high risk patients for additional support.
• Serve as a patient advocate and promote informed decisions.
• Participate in rounds and conferences to review goals and discharge planning.
• Collaborate with staff to develop and execute the plan of care.
