Job summary

Universal Health Services is hiring a Case Manager to support Southwest Healthcare Inland Valley Hospital in Wildomar and Rancho Springs Hospital in Murrieta, CA. The role focuses on assessing patient needs, coordinating discharge planning, and applying sound clinical judgment within the nursing process.

The ideal candidate has 3+ years of acute care experience, RN licensure in California, and exposure to Case Management or Utilization Review. CCM/CPUR certifications are preferred.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience in Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Identify and prioritize need for further assessment based on patient presentation and care setting.
• Initiate discharge planning upon admission as appropriate.
• Follow nursing process with sound judgment and regulatory compliance.
• Float to the other Southwest Health facilities based on operational needs.
