Job summary

Southwest Healthcare Rancho Springs Hospital in Murrieta, California, seeks a Registered Nurse Case Manager to coordinate patient care, initiate discharge planning, and ensure compliance with care standards. The role supports acute-care patients across the hospital and may require floating to Inland Valley Hospital as needed.

Candidates should have a minimum of three years of acute care clinical experience and RN licensure in California; CCM/CPUR certification is preferred.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred
• Completion of an accredited Registered Nursing program.

Responsibilities

• Identify and prioritize the need for further assessment based on patient presentation/diagnosis.
• Initiate discharge planning upon patient admission as appropriate.
• Demonstrate sound judgment and follow nursing processes and hospital policies.
• May float to the other Southwest Health Rancho Springs and Inland Valley Hospitals as needed.
