Southwest Healthcare Inland Valley Hospital in Wildomar, California seeks an experienced RN Case Manager to assess patient needs, initiate discharge planning, and support care transitions. The role emphasizes sound nursing judgment, regulatory compliance, and collaboration with the care team to optimize patient outcomes.

Formación

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsabilidades

• Identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting and responses to prior care.
• Initiates discharge planning for each patient upon admission as appropriate.
• Demonstrates sound judgment and effective decision making in following the nursing process.
• May float to the other facility within Southwest Health Rancho Springs and Inland Valley Hospitals based on operational needs.
