Job summary

Southwest Healthcare Inland Valley Hospital in Wildomar, CA is seeking an experienced registered nurse to join the Case Management team. The role focuses on identifying patient needs, prioritizing assessments, and initiating discharge planning for admission, with emphasis on the nursing process and collaboration with care teams.

You will work under UHS policies, support core values, and may float between Inland Valley and Rancho Springs facilities as needed.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting and responses to previous care.
• Initiate discharge planning for each patient upon admission as appropriate.
• Demonstrate sound nursing judgment and decision making while following the nursing process.
• Comply with regulatory, professional, legal and hospital policies and uphold core values and hospital mission.
