Job summary

Universal Health Services is seeking an experienced Case Manager for its California network, including Inland Valley and Rancho Springs Hospitals. The role focuses on identifying discharge needs, coordinating care, and ensuring smooth transitions for acutely ill patients.

You will float between facilities as needed, adhere to hospital policies, and demonstrate judgment in the nursing process. California RN license and CCM/CPUR certification preferred.

Qualifications

• Minimum three years of acute care clinical experience.
• Two years' experience in case management or utilization review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Case Managers identify and prioritize need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care.
• Initiates discharge planning for each patient upon admission as appropriate.
• Demonstrates sound judgment and effective decision making in following the nursing process. Case Managers will also practice in accordance with all regulatory, professional, legal and hospital policies as well as consistently demonstrate behaviors that model the Southwest Healthcare Rancho Springs and Inland Valley Hospitals’ Core Values and the hospitals mission statement.
