Job summary

Southwest Healthcare Inland Valley Hospital in Wildomar, California, is seeking a Case Manager to coordinate discharge planning and care for patients across the Inland Valley and Rancho Springs campuses. The role emphasizes clinical assessment, collaboration with physicians, and adherence to regulatory and hospital policies.

Qualified candidates will have at least three years of acute care nursing experience, RN license, and preferred CCM or CPUR certification.

Qualifications

• 3+ years of acute care clinical experience.
• 2+ years experience in Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care.
• Initiate discharge planning for each patient upon admission as appropriate.
• Demonstrate sound judgment and effective decision making following the nursing process while adhering to regulatory, professional, and hospital policies.
