Job summary

Universal Health Services seeks a Registered Nurse Case Manager to join hospital care teams in California. The role focuses on identifying patient needs, initiating discharge planning, and coordinating care per nursing processes and regulatory standards.

Applicants should have 3+ years of acute care clinical experience, with preferred case management or utilization review background and a California RN license; CCM/CPUR certifications are advantageous.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Identify and prioritize need for further assessment based on patient presentation/diagnosis and care setting.
• Initiate discharge planning for each patient on admission as appropriate.
• Demonstrate sound judgment and follow nursing processes; adhere to policies and regulatory requirements.
• Float to another Southwest Health facility as needed with proper orientation.
