Job summary

Providence Queen of the Valley Medical Center in Napa, CA, seeks a dedicated RN Case Manager for a full-time day shift. You will plan, coordinate, and negotiate care for complex patients across the continuum, supporting families and optimizing utilization.

This role emphasizes discharge planning, chronic disease management, and Utilization Management as needed. A California RN license and 2 years inpatient or outpatient case management experience are required, with a Bachelor's in Nursing

Qualifications

RN with coursework or training in case management.
2 years acute care hospital clinical experience.
1 year inpatient or outpatient case management experience.
California RN license required on hire.
2 years in case management, discharge planning, or utilization management in relevant settings.

Responsibilities

Plan, coordinate, negotiate, procure and manage care for complex patients and families.
Assist patients in utilizing appropriate health care services and manage the continuum of care.
Involvement in Utilization Management and Discharge Planning as needed.
