Job summary

Providence Queen of the Valley Medical Center in Napa, CA, seeks a dedicated RN Case Manager for a full-time day shift. You will plan, coordinate, and negotiate care for complex patients across the continuum, supporting families and optimizing utilization.

This role emphasizes discharge planning, chronic disease management, and Utilization Management as needed. A California RN license and 2 years inpatient or outpatient case management experience are required, with a Bachelor's in Nursing

Care Manager RN at Providence Queen of the Valley Medical Center in Napa, CA. This position is Full time and will work 8-hour, Day shifts.

The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Position may require Utilization Management and Discharge Planning, Chronic Disease Case Management based on inpatient settings.

Required Qualifications:

• Coursework/Training: Recent completion of a post-graduate level curriculum focusing on case management or :
• Current clinical experience with 2 years clinical experience in an acute care hospital setting, or
• 1 year current experience in an inpatient or outpatient case management setting.
• California Registered Nurse License upon hire
• Two (2) years case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.

Preferred Qualifications:

• Bachelor's Degree: Nursing. Required to obtain within 3 years of hire date
• Three (3) years Acute Care Experience

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
