Providence Queen of the Valley Medical Center in Napa, CA is seeking an experienced RN Case Manager to coordinate care across the continuum. This full-time role requires day shifts and emphasizes utilization management, discharge planning, and collaborative care with patients and families.

As part of a dedicated Providence team, you will assess complex cases, arrange services, and help patients navigate the health system while upholding our commitment to compassionate, individualized care.

Qualificações

• Recent completion of a post-graduate level curriculum focusing on case management.
• 2 years clinical experience in an acute care hospital setting, or 1 year in case management.
• California RN license required upon hire.
• 2 years case management, discharge planning, or utilization management in acute/sub-acute/home health or managed care.

Responsabilidades

• Carry out effective case management across the continuum of care.
• Plan, coordinate, negotiate, procure and manage care for complex patients and families.
• Assist patients in utilizing appropriate health care services and potential utilization management.
• May require discharge planning and chronic disease case management in inpatient settings.
