Job summary

Providence Redwood Memorial Hospital is seeking an RN Care Manager for a Per Diem, 8-hour Day shift position in Fortuna, CA. Qualifications

• Associate degree or nursing diploma required.
• CA RN license must be active at hire.
• 2 years of acute care in Med-Surg or inpatient setting.
• 1 year in care management or utilization review, or completion of TIPS/CM Orientation upon hire.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Coordinate daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization and efficient care delivery.
