Job summary

Boost Home Healthcare is seeking an RN Case Manager to administer skilled nursing care to patients in their homes on an intermittent basis, under physician orders and the plan of care. The role supports recovery and wellness, guided by the Director of Patient Care Services.

Compensation ranges per hour, with a strong emphasis on compassionate, patient-centered care. Qualified candidates will be a graduate of an accredited nursing program, currently licensed in the posting state, with at least

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as RN in the state of the job posting.
• Minimum of one (1) year of nursing experience in a home health setting.
• Must have current BLS Certification.
• Access to reliable transportation.

Responsibilities

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
