Boost Home Healthcare - Northern Colorado is seeking a compassionate Registered Nurse to provide patient-centered home health care in Colorado. The role emphasizes independence for patients at home and a supportive, family-like team environment.

Responsibilities include initial and ongoing patient assessment, plan-of-care initiation and revisions, and timely documentation. Competitive pay and comprehensive benefits accompany this position.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of one year nursing experience in a home health setting.
• Must have current BLS Certification.
• Access to reliable transportation.

• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

Job Type and Pay:

PRN, Part time and Full-time

• $43-$52/hr
