Job summary

Boost Home Healthcare in Fort Collins, CO is seeking licensed registered nurses to provide in-home care, perform initial evaluations, and regularly reassess nursing needs to support patient independence and recovery. We offer PRN, part-time and full-time roles with flexible day shift schedules.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in the state of employment.
• Minimum of 1 year nursing experience in home health.
• BLS certification required.

Responsibilities

• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

What we’re looking for:

• Day shift
• Monday to Friday 8am-5pm
• Weekends as needed
