Job summary

Boost Home Healthcare is seeking a registered nurse to serve as a Case Manager, administering skilled nursing care for patients in their homes in accordance with physician orders and plans of care. You will work under the Director of Patient Care Services and coordinate ongoing assessments and revisions to care plans.

The role requires a valid RN license in Michigan, a minimum of 1 year home health experience, current BLS certification, and reliable transportation.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of job posting.
• Minimum of one year of nursing experience in a home health setting.
• Current BLS Certification required.

Responsibilities

• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
